Dr. Bahareh Sabzehei received her periodontal training at the University of Illinois at Chicago after completing her dental training at UIC. She also received he Master of Science degree in Oral Sciences at the university of Illinois at Chicago. Her research was focused on factors affecting oral wound healing. She is proficient in treating all aspects of periodontal disease, periodontal plastic surgery, hard and soft tissue regeneration and implant dentistry. She is also a part-time clinical assistant professor at the university of Illinois at Chicago. Dr Sabzehei is committed to providing excellent patient care and life-long learning. Dr Sabzehei lives in Chicago area and enjoys exploring shows and concerts in the Windy city. She also enjoys running, biking and playing tennis.

dr. robert a. katin

Dr. Katin attended the University of Pennsylvania Dental School where he received his DDS in 1961. He completed his residency at Mt. Carmel Mercy Hospital in Detroit.
Dr. Katin is a Diplomate of the American Board of Oral and Maxillofacial Surgery and also participates in the Chicago Dental Society, Illinois State Dental Society, American Dental Association, American Association of Oral and Maxillofacial Surgeons, and the Academy of Osseointegration.
Dr. Katin and his wife, Marcia, enjoy spending time with their children and grandchildren. He also enjoys barbershop singing with the New Tradition Chorus, which won the International Gold Medal in 2001.

Dr. Lubar attended the University of Wisconsin where he earned his Bachelor of Science in 1964. He continued his education at Loyola University Dental School, earning his DDS in 1968. In 1972 he completed his residency in Oral & Maxillofacial Surgery at the University of Pennsylvania & Presbyterian Hospital. He began his private practice in surgery in 1972.
Dr. Lubar has been active in research in Dental Implantolgy. He has lectured extensively throughout the United States. He formerly taught at the University of Illinois Oral and Maxillofacial Department, Michael Reese Oral Surgery program, Mt. Sinai and Evanston Hospitals.
Dr. Lubar’s professional affiliations include the American Dental Association, the Illinois Dental Society, the Chicago Dental Society, the American Society of Oral and Maxillofacial Surgery and the illinois Society of Oral and Maxillofacial Surgery. He is also a Diplomate for the American Board of Oral and Maxillofacial Surgery and the American Society of Dental Anesthesiology. Dr. Lubar and his wife, Julie, enjoy travel, golfing, fishing and reading. He has two sons, Jason and Jamie, and a daughter-in-law, Jennifer. He loves “J” names.

dr. j. m. chawla

Dr. Chawla attended King George Medical and Dental College, Lucknow, where he earned his DDS and MS in Orthodontics. He has a Certificate of Speciality in Orthodontics from Loyola University and earned his DDPH from the University of Toronto College of Dentistry. He is currently affiliated with the Midwestern Society of Orthodontists, the Illinois Society of Orthodontists, the World Federation of Orthodontists and is a member of the American Association of Orthodontists. Dr. Chawla was formerly a Clinical Instructor, Teacher, and Professor of Orthodontics at King George Medical and Dental School, Lucknow; College of Dentistry, University of Oklahoma; School of Dentistry, Loyola University Chicago; and School of Dentistry, University of Illinois, Chicago. He is a licensed specialist in Child and Adult Orthodontics, T.M.J. and Orofacial Pain, and a Certified Invisalign® Orthodontist. Their daughter, Shalini, is an M.D. and Psychiatric resident at Chicago MedicalSchool/VA Hospital, and their son, Sumit, is a first year dental student at Tufts.

dr. bradford r. johnson
Dr. Johnson completed his undergraduate education at the University of Colorado and received his DDS degree from Virginia Commonwealth University. Dr. Johnson completed a one-year general practice residency at Michael Reese Hospital and practiced general dentistry for several years prior to endodontic specialty training at the University of Illinois at Chicago. He has been teaching in the Department of Endodontics since 1991 and became Director of Postdoctoral Endodontics in 1997. In 2005, he received a Master of Health Professions Education degree from the University of Illinois at Chicago.
Dr. Johnson has published clinical and research articles in various dental journals and is
coauthor of two textbook chapters. He is a Diplomate of the American Board of
Endodontics and a member of the American Association of Endodontists, Illinois
Association of Endodontists, American Dental Association, Illinois State Dental Society,
Chicago Dental Society and Edgar D. Coolidge Endodontic Study Club.
Dr. Johnson has lived in Lake County for over 20 years. In his free time, he enjoys fishing, travel and other activities with his wife and three children.
